Basal implantology offers treatment for patients with complete or partial edentulism and allows the restoration of entire jaws or segments of missing teeth in just 4 steps over 3 to 5 days.
The surface of basal implants is smooth with a special coating that prevents the retention of bacteria and the development of peri-implantitis. This significantly reduces the risk of inflammation around the implants.
This type of implant is used for patients at risk of losing a large number of teeth over time, or even all their teeth, such as in cases of severe periodontitis. They are also suitable for restoring individual missing tooth segments.
Visit 1
The basal implants are placed via surgery immediately after tooth extraction, and an impression is taken to fabricate bridges on the implants. The entire surgical stage of implantation is performed using a minimally invasive technique.
Visit 2
A trial fitting of the metal framework on the implants is carried out – 48 hours after implantation.
Visit 3
A trial fitting of the structures with ceramic placed on the metal framework is carried out; the contacts between the crowns, and the colour and shape of the ceramic crowns are checked.
Visit 4
The bridges (crowns) are cemented within 72 hours of implantation and the patient can eat immediately. For this reason, they are also known as immediate implants with immediate loading for 3 days. A post-implantation check-up is scheduled after the procedure.

Smoking reduces the quality of blood supply to the jawbones, which is an obstacle to recovery with dental implants. This leads to a delay in the healing process and a higher risk of gingivitis and gingival recession, which in turn leads to peri-implantitis. In smokers, osseointegration may be compromised, which can jeopardise the overall restoration.
With immediate-load basal implants, fixation occurs in the deep, hard parts of the bone, which are less well-supplied with blood and therefore less affected by the influence of nicotine and other harmful factors associated with smoking.

“Periodontitis and implants” is another widely discussed topic. In patients with severe gum disease – periodontitis – treatment with conventional (two-piece and rough-surfaced) dental implants may be unsuccessful.
This is mainly due to the higher risk of gum infections around the implants, leading to peri-implantitis and, in the final stage, to the loss of conventional implants.
The main symptoms and consequences of developing periodontitis experienced by patients include bleeding and pain in the gums and mouth in general, difficulty eating, noticeable bone loss, loosening and wobbling of the teeth, and eventually their complete loss. This inevitably leads to a number of risks for the body and the patient’s overall health.
Firstly, periodontitis can increase the risk of cardiovascular disease, as well as create conditions conducive to a range of lung infections. It is precisely in such patients that immediate basal implants are used with great success.
During this procedure, all of the patient’s teeth that are a source of infection are removed and immediately replaced with smooth-surfaced implants.
Within just 3 days of the procedure, patients have new teeth (crowns) on basal implants and can smile and eat normally. If necessary, gum reshaping is also performed.

Conventional dental implants are usually contraindicated for treatment in patients with diabetes. The main reason is that diabetes is associated with comorbidities, including increased susceptibility to infections, impaired wound healing, gingivitis, periodontitis and other disorders of the masticatory system and gums.
Basal implants, however, yield very good results in patients with controlled diabetes, because their smooth surface reduces the likelihood of a bacterial infection developing over time. Furthermore, the bearing surface is located in the deep cortical bone, which is far from the areas of the crestal alveolar bone where bacteria usually attempt to invade.

When it comes to the disadvantages of basal implants, the financial aspect is a key consideration. As an implant procedure, basal implants are more expensive compared to removable dentures. However, the long-term benefits and comfort justify the investment in high-quality treatment.
The risks associated with basal implants are minimal, but in some individual cases, even the possibilities of strategic implantology and the placement of basal implants are not feasible.
Problems with basal implants may arise, for example, in patients taking bisphosphonates – medicines used to treat bone loss. The risk of complications with basal implants in such cases requires an individual approach and a detailed assessment by the specialist before treatment begins. Another factor is teeth grinding (bruxism).
